List Of Documents Required To Buy Health Insurance

    4. Driving Licence
  • Just so you know, PAN card is no longer acceptable as identity proof.

  • Proof of Age

Age proof is another mandatory document to be submitted for health insurance. Any of the following can be submitted as proof of age:

    1. Aadhaar Card
    2. Passport
    3. Voter ID
    4. PAN
    5. Birth certificate
    6. Driving Licence
  • Proof of Address

This again is an essential document required for health insurance. It may also be used by your insurance provider to communicate with you on your postal address. Some of the documents that can be used as an address proof are mentioned below:

    1. Driving Licence
    2. Ration Card
    3. Aadhaar Card
    4. PAN
    5. Passport
    6. Rent agreement (if applicable)
    7. Utility bills like electricity, telephone, etc.
  • Passport Size Photograph

Passport size photo is again used with your identity. This may be optional.

  • Medical Reports

Medical reports are demanded as per the insurer’s requirements. These are generally needed to access the current health condition and pre-existing diseases (if any) of the policyholder. 

  • Proposal Form

This document shall be duly filled and signed in. Health insurance is important for you and your family at any stage in life.
